Not seen this before, ever. And I cannot recreate it either, but knowing it appeared may give you a clue:
I had 3 tabs open, none of them had been modified, and pressed CTRL+HOME (mapped as !UP MAX) on one of them.
I dismissed the pop-up window, switched to another tab and press CTRL_HOME there - same result
Checked KEYMAP to be sure CTRL+HOME did the right thing - setting was correct.
I closed SPFlite and reopened it and the issue went away.
Hope this makes some sense to you.

Robert is quite right. I've never been able to locate this. But I'm sure it's not the UNDO code itself, UNDO is the innocent victim of some memory trampling by something else.
Now, how to find that 'something else' that's the real culprit?
Frankly I've no idea right now, it has to be some series of actions preceding the pop-up, but nothing repeatable has ever been found/reported.
